WAIVER OF LIABILITY

I, {name}, in consideration of being allowed to participate in the training and activities at MMA Essential (the "Gym"), acknowledge, understand, and agree to the following:

1. Assumption of Risk:

   I acknowledge and fully understand that martial arts training and physical exercise involve inherent risks and dangers, including but not limited to serious injury, permanent disability, and death. I voluntarily assume all risks associated with participating in the activities at the Gym, including but not limited to training, sparring, and using the Gym's equipment and facilities.

2. Waiver of Liability:

   I hereby release, waive, discharge, and covenant not to sue the Gym, its owners, instructors, employees, agents, and representatives (collectively, the "Released Parties") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in any activity at the Gym, regardless of whether such loss, damage, or injury is caused by the negligence of the Released Parties or otherwise.

3. Medical Clearance:

   I certify that I am in good health and have no medical condition that would prevent me from safely participating in the activities at the Gym. I understand that it is my responsibility to consult with a physician and obtain medical clearance prior to engaging in any physical activity at the Gym. I agree to provide a medical clearance certificate upon request.

4. Membership Responsibility:

   I understand that it is my responsibility to manage my membership with the Gym. If I wish to cancel my membership, I must do so either online through the Gym's website or by speaking directly with a team member. Failure to properly cancel my membership may result in continued billing, for which I will be responsible.

5. Behavioral Conduct:

   I understand and agree that the Gym is a place for growth and development. Any tension or conflict that arises during training or sparring sessions should not be carried outside of the Gym. I agree to maintain a respectful and positive attitude towards other members, instructors, and staff at all times.

6. Use of Facilities:

   I agree to follow all rules and guidelines set forth by the Gym regarding the use of its facilities and equipment. I understand that failure to comply with these rules may result in the termination of my membership.

7. Indemnification:

   I agree to indemnify and hold harmless the Released Parties from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney's fees, arising out of or related to my participation in the activities at the Gym.

8. Media Release (Optional):

   I understand that the Gym may take photos and videos during training sessions and events for promotional purposes on social media and other platforms. I have the option to opt-out of appearing in any such media by checking the box below:

I do NOT wish to be on camera or have my image posted on social media.

By electronically signing below, I acknowledge that I have read, understood, and agree to the terms of this Liability Waiver and Membership Agreement. I am aware that by signing this agreement, I am waiving certain legal rights, including the right to sue the Released Parties
